For great care at the best price, go in network!

Whether you have the Standard or Enhanced Plan, you can choose a dentist from either the Delta Dental PPO or the Delta Dental Premier network. Plan payments will be the highest (and your payment responsibility the lowest) when you choose a dentist in the Delta Dental PPO network. The Premier network provides lesser discounts with a larger provider selection.

Benefits of seeking in-network care include:

  • Higher discounts: dentists have agreed to rates often lower than their usual fees.
  • No balance billing: you won't be billed for the difference between the procedure cost and the plan's covered amount.
  • No paperwork: in-network dentists will take care of your claims and will be paid directly by Delta Dental.



Out-of-network claim form

When you visit an out-of-network provider, you may need to submit your own claims.
