Delta Dental is the leading provider of dental benefits in Minnesota. It offers comprehensive dental plans and access to the largest dental networks in Minnesota and across the nation. Delta Dental also offers significant network savings, complemented by world-class service you expect and deserve.

• Delta Dental PPO (Base Plan for Twin Cities/Duluth) is an affordable, network-only plan, offering strong cost savings. This program is considered the base plan for many employees. Under this plan, you must visit a dentist participating in the Delta Dental PPO network to receive benefits. With the exception of out-of-area emergencies, benefits will be denied if you do not visit a Delta Dental PPO dentist.
  
  The Delta Dental PPO network includes 1,514 participating dentists in 72 Minnesota counties. This includes access to our statewide specialty network. With a Delta Dental PPO dentist, your out-of-pocket costs may be reduced.
  
  Confirm if your dentist is participating in the Delta Dental PPO network or consider if you are willing to change dental providers, prior to enrolling for this dental plan. Remember, this is an in-network only program.
  
  This program offers in-network access to a select number of providers at the following locations: Boynton Health Service, U of MN Faculty Practice, U of MN Dental Clinic. Please check our website for a list of PPO dentists at these locations.
  
  NOTE:  Not all providers at these locations are contracted under Delta Dental PPO. Please refer to the Dentist Search on this web site, contact Customer Service or review a provider directory for a complete listing of providers at these locations.

• Delta Dental Premier is a flexible plan that offers access to the broad Delta Dental Premier network as well as the more cost-effective Delta Dental PPO network. Out-of-network benefits are also available. The Delta Dental Premier network is Minnesota's largest dental network - 2,659 dentists in 86 Minnesota counties and border communities. The Delta Dental PPO network includes 1,514 participating dentists in 72 Minnesota counties. Visiting a dentist in either of the Delta Dental networks will help you make the most of your benefits and can result in out-of-pocket cost savings. Choosing a dentist in the Delta Dental PPO network may result in greater cost savings. As an added convenience when you use a participating dentist, the dentist files the claim for you, and we pay the dentist directly.

• University Choice is an open access program and offers freedom to see any dentist of your choice. Reimbursement for services not provided by a Delta Dental network provider will be paid upon a scheduled table of allowance. There may be a payment difference between the allowed reimbursement and what the dentist charges for a particular service, which could result in greater out-of-pocket costs for you. You may be required to file your dental claims if you are a member of this program, and all payments will be made directly to you, not your dental provider.
