Your ID Card

You will receive an identification card in the mail within 7-10 business days if you are a new Delta Dental member or if you have a life event (e.g. marriage, divorce, birth of a child), which requires a new card. It is important to bring your ID card to your dental visits. To order a new ID card, call Delta Dental's Customer Service at (651) 994-5436 or (800) 465-8953 (toll-free).

About 3M's Dental Plan

Delta Dental of Minnesota is the claims administrator for 3M’s dental plan and provides access to the Delta Dental PPO (formerly DeltaPreferred Option) and Delta Dental Premier (formerly DeltaPremier) network of dentists. 3M’s dental plan is a “silent” Preferred Provider Organization (PPO), which provides discounts for services. With a “silent” PPO, you can visit any dentist inside or outside the network and receive the same benefit level from the plan. However, visiting a dentist in the Delta Dental PPO or Delta Dental Premier network may save you money. In-network dentists agree to charge competitive, discounted rates. If you are an active 3M employee, find details about what is covered under the 3M Dental Plan by visiting 3M Go or Your Benefits Rewards™.

3M Retirees

3M Retirees group coverage ended on December 31, 2015. 3M Retirees can enroll in a dental plan that closely resembles your current 3M Retiree plan - or choose a plan that may align more closely with your unique needs or budget. With continued coverage, most Delta Dental Individual and Family Dental Plans will waive the waiting period so you don't have to worry about gaps in your coverage.

Dental Claims

When you visit a Delta Dental PPO (formerly DeltaPreferred Option) or Delta Dental Premier (formerly DeltaPremier) dentist: Show your identification card to your dentist who will process your claim so you don't have to worry about any paperwork. If you visit a dentist outside the Delta Dental PPO or Delta Dental Premier network: Show your ID card to your dentist and ask if you need to submit your own claims. To file a claim, use this form.

  1. Can I continue to visit my current dentist?
    Yes. Under 3M's dental plan, you can visit any dentist, even if he or she is not in the Delta Dental network. However, visiting a dentist in the Delta Dental PPO (formerly DeltaPreferred Option) or Delta Dental Premier (formerly DeltaPremier) network may save you money.
  2. What is a “silent” PPO?
    A Preferred Provider Organization (PPO) is a network that provides discounts for services. Our dental plan is a “silent” PPO, which means you can still visit any dentist. The kinds of services covered under the plan and percentage the plan pays for covered services are the same whether you see an in-network or out-of-network dentist. However, you may want to consider using a dentist in Delta Dental's network, because you and 3M may save money.
  3. How much will I pay if I go to an in-network or out-of-network dentist?
    Refer to the 3M Dental Plan Summary Plan Description (SPD) for more detail on the different cost calculations when using an in-network or out-of-network dentist.
