Delta Dental PPO Plus Premier (Voluntary Network Program - Dental Flex) and Voluntary Non-network Program - Discover

Discover and Dental Flex are our most affordable plans for employers. These voluntary plans are great options for groups that want to give employees access to a dental benefit plan while controlling the company’s costs and contributions. In a voluntary plan, employees make premium payments, and the employer can contribute as much or as little as they want toward those payments – or nothing at all. Delta Dental PPO^SM
The Delta Dental PPO program is our network plan with the strongest coverage and greatest opportunity for savings, including access to the state’s two largest dental networks – the Delta Dental PPO network and the Delta Dental Premier network. Read our Delta Dental PPO Brochure for more information. Delta Dental PPO has about 1,500 participating dentists in Minnesota and more than 74,000 participating dentists across the nation. Delta Dental Premier has more than 2,650 participating dentists in Minnesota and more than 134,000 dentists nationwide.

Delta Dental PPO–Lifetime Deductible and Escalating Maximum^SM
This competitively priced plan for groups of 5-99 lives offers significant long-term cost savings. In addition to a lifetime deductible, this Delta Dental PPO plan includes an escalating annual maximum that starts at $1,000 and increases by $200 each year until it reaches $2,000. Orthodontic Discount Program
The Orthodontic Discount Program is an alternative for groups that do not purchase traditional orthodontic coverage. With the discount program, members receive guaranteed discounts at dentists in our Twin Cities area orthodontic network. Employers may purchase program membership for their employees, or they can make membership available to employees on a voluntary (employee-paid) basis. To request a benefit summary for groups of 5-199 employees, please e-mail deltadentalconnect@deltadentalmn.org. Existing groups, please provide us with your group name or group number. For new groups, please provide us with the product name, deductible, orthodontics type (traditional or orthodontic discount program, if applicable) and maximum.
